Supporting families, empowering futures
Team Continue Foundation is a non-profit organization dedicated to providing financial assistance for outpatient treatment and care for families of adolescents and young adults who have been treated for suicide attempts or ideation. Our foundation supports local suicide prevention charities and offers an annual academic scholarship. Founded in February 2021, we are committed to making a positive impact and providing critical support to families during the period between in-patient care and effective outpatient treatment solutions. Join us in our mission to prevent suicide and support families in overcoming the challenges presented by mental health issues.
Our Services
-
Therapy Grants
We provide access to therapy for teens and young adults who lack the financial means or support to receive critical mental health care. Many of our grant recipients face extraordinary challenges, including past trauma, suicidal ideation, and severe mental health conditions, often without insurance or a support system.
-
Advocacy and resources
We actively work to address the barriers preventing individuals from accessing the care they need. From connecting recipients with transportation and housing solutions to addressing food insecurity and financial gaps, we help those navigating life with the odds stacked against them. Your support enables us to advocate for mental health equity and provide essential resources to those who need them most.
-
Academic Scholarships
Each year, we award an academic scholarship to a deserving Rochester Community Schools (RCS) student who has demonstrates resilience. This scholarship supports students as they pursue higher education, empowering them to overcome challenges and build brighter futures. By investing in education, we help create opportunities for personal and professional growth that can transform lives.
Real lives, real impact.
At Team Continue, our mission is rooted in compassion, action, and support for those navigating mental illness, trauma, and suicidal ideation. Thanks to the generosity of our donors and community partners, we provide funding for therapy and mental health services when other options fall short—or don’t exist.
These are just a few of the stories from grant recipients whose lives have been touched by your support:
-
After years of emotional and physical trauma and being disowned by his family, a 23-year-old man is doing his best to live independently. With no support system and limited resources, Team Continue helped fund therapy sessions to ensure he wasn’t alone in facing the weight of his past.
-
At just 15 years old, this young man has cycled through inpatient care six times in nine months. Without medical insurance, continued support seemed out of reach. Our grant funding allowed him to access therapy post-discharge, giving him a fighting chance to break the cycle.
-
Living with muscular dystrophy and a combination of mental health challenges, a 21-year-old woman found herself without family support or stable housing. With care coordination between a psychiatrist and a case manager, our support is helping her maintain mental health stability—and a safe place to heal.
-
A 22-year-old woman was denied access to therapy by her own family due to cultural stigma, despite a long history of depression, suicidal ideation, and multiple attempts. With no transportation and limited income, she continued seeking help. Team Continue stepped in to fund her counseling so she wouldn’t have to fight alone.
